Wow. The work that those people do is so complex and technical, it's like asking a philosophy student to do brain surgery.
This isn't a new idea. starting after the Civil War and into the 1930s there were peonage camps. Sheriffs, particularly in the South, would snatch people off the street for vagrancy, a judge would convict them, and they would be stuck in them as slave labor.

Anyone who didn't see this coming would have had to be blind and deaf. I think that the fear of this drove a lot of the reactionary sentiment that got channeled into Trump's re-election (even though he would be the first person to screw them over).
If he really believes he is dying, his narcissism is enough that it will enrage him that others will live and he will do everything he can to see that that is not the case.
You are assuming that he looked better than this in real life, while in reality, he may have looked much worse.
