I think part of the cause is that men can put on more muscle proportional to body mass, meaning they tip a balance of weight/strength.

Carrying that much muscle bulk slows them down. Whereas with women, they can't put on as much muscle as easily, and so they retain speed.

I can imagine for some, it feels like victim blaming when the responsibility is placed on the cyclist instead of the drivers.

I once hit a cyclist (they were fine) because they had no lights and came from the side at night, meaning my headlights could not have seen them. They were absolutely invisible to me.

It does happen. Visibility does help. But the main issue is bad drivers, aggressive drivers and impatient drivers.
