It's staggering that, while a number of Service Personnel and contractors' staff were aware of significant issues over the entire programme, MoD still haven't realised the value of an independent disclosure platform. We've a webapp that can just do that for less than £6bn.

There are a significant number of Veteran Aware Trusts unable to meet Accreditation Standards Six and Seven, because staff are unaware of the requirement or the Trust doesn't have the required mechanisms.
