If distribution and dissection of LA good, i make only one injection pro side in superficial approach. And if there are multiple subxiphoid drains, high rectus sheath block for that

It could be game changer in analgesia in cardiosurgery.
I use parasternal blocks for every patient on icu after sternotomy. I started with superficial, but i find analgesia isn’t predictable and long lasting. Now i use only deep, i find it much better.

Es gibt gar nichts im Internet dazu. Ich habe so entschieden, falls Candidämie in BK Kontrolle persistiert, dann PDK ex. Die folgende BK waren negativ.

Evidence dabei ist nicht so überzeugend und man braucht dafür TDM. Bedeutet in DE in vielen KH nicht möglich

LA selb haben antibakterielle Wirkung. Lidocain sowohl gegen grampositive als auch gramnegative Keime. Generell bei single shot Infektionsrate ist sehr niedrig.

Was auch nicht schlecht ist. Was sehr in DE unterschätzt ist, mein Lieblingsblock bzw Katheter paravertebraler. Fascial blocks sind cool und so fancy, aber Evidence is nicht so überzeugend.

Ich bin großer Fan von RA, würde gerne mehr machen aber meistens ist leider nicht von mir abhängig. In einiger KH, wo ich gearbeitet habe, sind Chirurgen gegen RA. RA mit Vollnarkose verliert viele ihre Vorteile, Sedierung reicht vielen Pat nicht.

Ich bin großer Fan von RA, würde gerne mehr machen aber meistens ist leider nicht von mir abhängig. In einiger KH, wo ich gearbeitet habe, sind Chirurgen gegen RA. RA mit Vollnarkose verliert viele ihre Vorteile, Sedierung reicht vielen Pat nicht.
Man soll auch hier sagen, dass die Mehrheit von Patienten hier dagegen sind. „Ich will tief schlafen und nichts mitkriegen“ ist Hauptwunsch von denen.

Beides lernen/beibringen auf jeden Fall. Aber routinemäßig sicheres Verfahren verwenden. Wenn die Equipment erlaubt, dann VL immer. Wie es aktuell in DE aussieht, es soll niedrige Schwelle für VL geben. Aber routinemäßig bleiben wir noch trotzdem bei DL.
