Strong signal for better kidney biopsy adequacy favoring 16G needle over 18G (w/ = complication rate) in pediatric pts 12-18 years-old, extending established norm 4 adults. Only 4 children <12, a smaller 18G needle is safer. #KidneyBiopsyAcademy @kidneycon.bsky.social @arkanalabs.bsky.social

3/ By electron microscopy this case demonstrated numerous, randomly arranged fibrils that measured ~10 nm characteristic of amyloid fibrils within mesangial areas with extension into numerous capillary loops. Congo red stain was positive in glomeruli.

2/ This finding is classically seen with amyloidosis. Other glomeruli in this case demonstrated areas of PAS-weak to negative staining of mesangial areas which is another feature that should raise suspicion for possible amyloidosis.

1/ The light microscopic image of a Jones silver stain shows a glomerulus with demonstrates numerous elongated, fine spicules which some regard as the "eyelash sign".

However, the prominent IgG deposits also involving the associated arteriole in this case are an important clue that helps argue for a secondary etiology. This patient, in fact, was found to have systemic lupus erythematosus. (2/2)

This immunofluorescence (IF) image shows extensive granular glomerular capillary wall IgG deposits, consistent with membranous glomerulopathy. The glomerular staining pattern as seen by IF is not specific for either primary or secondary membranous glomerulopathy. (1/2)

Minimal change disease may show a subtle, finely granular, cytoplasmic IgG staining in podocytes (podocyte dusting) on immunofluorescence. This finding has been associated with podocyte injury mediated by nephrin autoantibodies.

Overall, the biopsy in this case demonstrated a dense granulomatous tubulointerstitial nephritis which after clinical exclusion of other causes (i.e. infection, drug/hypersensitivity) were compatible with the patient's known prior history of sarcoidosis.

At the end of the arrow is what is known as an asteroid body which is classically associated with sarcoidosis;
however, these are by no means specific for sarcoid as asteroid bodies have been seen in many other conditions including infections, various malignancies, foreign body granulomas, cysts,etc
