Great not only to have the work we're doing on HLH included in today's conference for the release of the #MBRRACEUK report on UK maternal mortality, but to also see another presentation on this topic. Is HLH as rare as we think, or simply rarely diagnosed?

- 104 pregnancies
- commonest cause infections (EBV, CMV, HSV, parvo)
- followed by SLE, stills, lymphoma
- 4/14 tested has positive genetics
- 40% no known cause

- AIHA, HELLP, & CAPS reported with HLH in this population
- of those presenting postpartum, most became unwell within a month of delivery
- 75% had a biopsy (bone marrow or liver), 20 negative for haemophagocytosis,10 had a repeat biopsy - all demonstrated haemophagocytosis

French Teratology Service study of 57 pregnant women exposed to anti-IL-1 (47% for Stills Dz) found no evidence of adverse pregnancy outcomes with IL-1 inhibition (43 anakinra, 5 canakinumab) 85% livebirths, 11% early miscarriages. Major congenital malformations 3% academic.oup.com/rheumatology...

Assessing risk of adverse pregnancy outcomes among women with SSc in the Swedish Medical Birth Register showed increased risk of preeclampsia, preterm birth, and caesarean delivery. Increased risk was also noted in the 3 years before SSc diagnosis
