I recall watching an episode of Freedom to Choose where Uncle Milt was bragging up the great healthcare system in Hong Kong, where it was apparently capitalist innovation in a free market economy that had produced another state-run system modeled after the NHS.
The late economist Milton Friedman said nothing is as permanent as a “temporary” government program. For 80 years, the health system has been warped by a decision during World War II to exempt employer-sponsored health insurance from wage and price controls. Employers used health benefits in lieu of wages to attract and retain workers. That’s how insurance started to become a standard feature of employment.

FRED's producer price index for window glass has increased by 75% since 2004 while the price of solar panels has dropped by 93.8%. They both use similar SiO2 feedstocks, maybe they're converging on that cost?

If I cannot walk, talk, eat, see, or hear with protruding brain tissue under a thin mucous membrane my caregivers and my family deserve your money, but I am a conservative so anyone else should work themselves to death turning tiny screws for Tim Apple if they want to see a doctor.
But David lived. We celebrated his fourth birthday in July. I do not understand the course of his life; I am simply grateful. David does not walk, talk or eat independently. He is visually impaired and has hearing loss. He has an unrepaired cleft palate open to protruding brain tissue, covered by a thin layer of mucous membrane. Developmentally, David is like a 10-month-old baby. He is our joy, and it is our privilege to parent him. I am a Medicaid provider because I am paid to care for my son, as are my son’s physical therapists, occupational therapists, primary care physicians, specialists, case managers and others. I expect my provider rate to be reduced by 8 percent effective Oct. 1 if no further action is taken by the state legislature to fully fund the North Carolina Medicaid program.

To illustrate, consider a woman whose life is endangered by her pregnancy. Is it morally permissible for her to have an abortion in order to save her life? The DDE says that an abortion is not permissible. Since abortion kills an innocent human being and since intentionally killing innocent human beings is always wrong, it is always wrong to have an abortion-even to save the woman's life. Abortion also fails condition 2 (the means-end condition). Killing the innocent in order to bring about a good effect is never justified, not even to save a whole city – or the world. As the Stoics said, “Let justice be done, though the heavens fall.” However, if the woman's uterus happens to be cancerous, then she may have a hysterectomy, even though it will result in the death of the fetus. This is because the act of removing a cancerous uterus is morally good (thus passing condition 1). The act of performing a hysterectomy also passes condition 3, since the death of the fetus is the unintended (though foreseen) effect of the hysterectomy. Condition 2 is passed because the death of the fetus isn't the means of saving the woman's life-the hysterectomy is the act that saves her life. Condition 4 is passed, because saving the woman's life is a great good, at least as good as saving the fetus. In this case, given the DDE, the woman is really lucky to have a cancerous uterus (rather than a pregnancy-related life-threatening condition)
