Optimal SCS enhances PAG and midbrain activity while reducing signals in emotional/cognitive hubs like the ACC and insula. On a network level it strengthens descending pain inhibition and modulates affective/cognitive pain pathways for effective analgesia. (3/4)

We saw that paresthesia-free or low-frequency (40 Hz) stimulation augments prefrontal engagement, while high-frequency (1200 Hz) modulates sensorimotor cortices. (2/4)
