Excited to speak at UT Houston-Methodist on Friday on "Innovative Clinical Trial Designs in ID"

First time I have been back to Houston in 26 years. It's still hot... and even more buildings on medical campus.

The new Dr. Fungus app is now available on all iOS and Android devices. This app serves as a handy companion to the Dr. Fungus website, providing quick, easy access to information on fungal organisms, medical mycoses, and antifungal medications. We will be continuing to improve and refine the app.

Two studies, same signal👇
📊 COVID-OUT RCT: 41% ↓ long COVID with 14d metformin (1500 mg/day by Day 6).
📊 CID UK cohort (766k pts): 64% ↓ long COVID if metformin started within 90d.

Cheap, safe, generic drug. Replication across RCT + real world = 🔥

Much more interesting and relevant would be the EPIC-SR results which showed no such benefits.

That would be more relevant data for today’s population in 2025 and beyond. @ofidjournal.bsky.social

VP shunts are key for CSF diversion in refractory coccidioidal meningitis. In a study of 124 patients, 17% had abdominal pseudocysts, often with shunt malfunction symptoms. 71% had active infection.##idsky
Trapped Below: Abdominal Pseudocysts Associated with Ventriculoperitoneal Shunts in Coccidioidal Meningitis
AbstractVentriculoperitoneal (VP) shunts are the mainstay for cerebrospinal fluid (CSF) diversion in patients with refractory coccidioidal meningitis (CM)-associated hydrocephalus. Abdominal pseudocysts (APCs), an uncommon but known complication of distal shunt catheters, have not been well described in CM. We conducted a retrospective study of 124 patients with CM who underwent VP shunt placement between 2010 and 2024. APCs occurred in 21 patients (17%), with most presenting with symptoms of shunt malfunction rather than abdominal complaints. This incidence is notably higher than previously reported in patients with hydrocephalus due to other etiologies. Evidence of active Coccidioides infection was present in 71% of patients. Imaging typically revealed fluid collections at the distal catheter tip, and most patients underwent surgical intervention targeting the distal shunt. Despite intervention, recurrent shunt failures occurred in a substantial proportion. These findings add to the limited literature on APCs in CM and may inform future research into their presentation, contributing factors, and management.

Probably the regular 100mg/kg higher dose for a shorter duration would be better utilization.

While survival was similar (in this small sample size n=48 vs n=171 from Ambition trial recruited in Uganda), re-hosptializations trended towards being likely higher.
