My highly reductive thinking is:

rsi- to go from awake and defending your airway to cuffed tube in trachea in the safest and shortest possible time.

This time is a bit on a spectrum, as a person barfing up poo probably needs the shortest possible time.

Thio, sux, cricoid etc. aren't magic. They were thought up by clever people, and new clever people get to think up the next version, and make it better. This is how things move on towards "optimal".

At the start of covid, I was doing an ABG for a pt under resp. Knew the recently-appointed consultant and she was like "you use local?".

She was a bit surprised by "yes because of *your* specialty's guidelines".
