Speaking from the Canadian experience, and in answer to your questions:

1) The biggest hospital "chains" serve the public just like all the rest.
2) No, they don't opt out. They can't.
3) In Canada, the provinces "run" healthcare.
4) Yes, I'm ok with having to see my GP before a specialist.

A perfectly valid point.

And also the reason why Congress should focus on addressing the myriad of stupid laws (or lack of laws) that Trump has so masterfully exploited, and which paved the way for his rise to power.

Will they? I highly doubt it.
