With this addition we can see when they're utterly afraid but it can lead to powerful moments where they're pushing past their fear and we can see it via their eyes but intrestingly enough the fear isn't fully gone so their eyes are still messy

And a addition i love to this is that in moments of genuine fear and distress for a character their eyes become messy as well and pitch black

I also love how fujimoto protrays distress and fear in the art for example when denji vomits blood from his heart condition the lines get sharper and messy making it feel different but still be the same artstyle while reflecting his state of mind
