Saw this in action recently with a near miss. First attempt was too low, barely (!) missed diaphragm. Second attempt (different provider) used POCUS, measured gaps, safe insertion. As a nurse, I will advocate for POCUS site confirmation from here on out (when available).
disagree with this “triangle of saftey” nonsense

in morbid obesity or cardiomegaly, this location could be dangerous (eg, diaphragm can be really high)

so POCUS is key to establish anatomy before tube insertion

(and incidentally, ICU patients never look like this in real life 🙄) #EMIMCC
