Yes and no. Can a therapist over-validate you, sure. However, there could be real licensing board type consequences for a therapist telling a person with psychosis "yes, your mom is totally poisoning you" or a suicidal person "here's how to do it" as LLMs have recently done.

There are a number of reasons someone might choose to pay out of pocket for a therapist, chief among them being, frankly, that they can afford it. A lot of specialized care is much more accessible if you can afford to pay out of pocket.

None of this is a commentary or statement on the way things should be. It is just an explanation of the way things ARE, and will likely continue to be as long as our healthcare continues to be mediated by large agencies that would prefer not to spend money treating people who don't "need" it.

If your therapist is being primarily paid by someone other than you, they are, at least officially, in the business of Treating Mental Illness and they are required to document and prove that that is what they are doing.

The most prominent therapists of the groovy "mental illness is just ~labels~ man, my job as your therapist is to ~walk beside you on this journey called life~" variety are generally able to be this way in their practice because they are paid directly by their patients.

It's because, if grief-related conditions are NOT in these manuals, then if your dad dies and your mom would like to talk to someone about her Normal Grief, she has to pay out of pocket. Her grief is Normal and there is nothing wrong with her worth treating.

So when we talk about, say, grief-related conditions being added to these manuals, that usually isn't because we are jonesing to pathologize grief and promote the view that nobody should be upset after being bereaved.

That means that generally speaking, if you're not paying out of pocket, your therapist HAS to diagnose you with something in order to justify spending Kaiser's/the taxpayer's/whoever's money on treating you.

Neither insurance companies nor most government-run healthcare systems are interested in paying for mental health care for people who are already mentally healthy. That is, from the perspective of a large bureaucracy trying to use its money efficiently, a huge waste of resources.

This is a great jumping-off point for something that I think a lot of people don't understand when we talk about adding certain things to the DSM or ICD.
