Don’t want people on hospice? A) You’re dumb. Hospice care is better than the alternative. B) Fund better acute rehab and SNF coverage, get more SNFs, and find a way to get patients out of the hospital other ways. Because your hospitals are drowning in patients rn with no hope in sight.

Instead, we get patients admitted to hospitals with no way for them to get out. They start churning out massive expenses and hospitals eat the cost. Hospice becomes the only viable way to get them out of the hospital.

Question 2 is why this is being over utilized. And that’s because our health system is utter dog 💩. If we had universal healthcare that covered nursing homes, and we had more available nursing homes, we wouldn’t need to be as aggressive at using hospice whenever possible.

So question 1 is if this program helps? Of course! It saves money by selecting people who don’t want all of this anymore, and stops churning out er visits and admissions that are super expensive.

Part 3 is they essentially choose to be palliative care only. Foregoing future expensive/invasive medical care. It’s a commitment to give up the ghost, rather than spin in and out of the system over and over.

The criteria is intentionally vague, so reasonable patients won’t get excluded. Essentially it asks if they have a “terminal illness” (super vague). It could just be advanced uncontrolled diabetes, not necessarily cancer. Part 2 is if the disease runs its normal course are they likely dead in 6m.

Ok, so the entrance requirement for hospice care is not 100% certainty of death within any given time period (say 6-12 months). Nobody knows when people will die. MDs aren’t asked to be fortune tellers.
