It was the first-line medication offered for me with POTS and BP that trends high! Some of the more common POTS meds increase BP (because ppl with syncope tend to have low BP) and that obvs won't work for hyperadrenergic-type POTS.

In a study of 47 patients with hyperadrenergiv POTS, 72% responded well to guanfacine, with improvements in fatigue, orthostatic symptoms, and a reduction in hyperadrenergic episodes.
