I have a potentially dumb pharmacology question- as I understand it, ASA at lower doses is selective for the linear pathway of eicosanoid metabolism, thereby preventing thromboxane and lipoxin synthesis, and there's a ceiling effect for the antiplatelet aspect that is achieved even at low doses.

Those are all important concerns to discuss w your patient when determining or changing a treatment plan. Bone loss from longterm PPI use increases the risk of injury from falls. Having an antiplatelet effect from NSAIDs and SSRIs means an increased risk of bleeding in a fall. It's the same issue.
