I’m amazed by this from a safety perspective.
There’s a thing called the Peltzman effect - deliberately accepting more risk because safety measures have been introduced.
Peltzman found people engaged in riskier behaviour and had more crashes when car seatbelts were introduced. #hfesky #safetysky
Our residents who have only trained on VL don't (or can't) position the patient properly and need to use a bougie most of the intubations. When it really gets tricky they'll struggle

Also DL works even if you have gunk in an airway that would cover the camera

Excellent discussion about a paper that implores us to stop researching and talking about 'safety culture'. It is a term that has stopped being useful and it is sucking time and energy away from things that can actually be effectively researched and improved.

Being a 'good rescuer' is:

• counterintuitive eg the advice to stand back & analyse rather than pile in & do something
• counter-cultural eg using checklists rather than relying on memory

Specific rescuer training would reinforce these skills.

What is a common thread in medical tragedies?

Rescuers fail to cut through the ‘mind lock’; they follow the primary caregiver down the wrong, fatal, path.

If we are going to train our way out of this problem, we need to train the rescuers.

Why do skilled practitioners make errors?

Mind lock - a rigid fixation on a diagnosis/course of action

Confirmation bias - contradictory data (eg abnormal capnography) are ignored, while corroborative information (eg breath sounds) is favoured

It’s likely that experts gain less in terms of reduction of physical stress due to awkward body posture than novices as they have refined technique over time.

Maybe though, training of DL & VL both need to emphasise body posture and physical ergonomics of intubation?

There’s definitely a difference in posture between novices and experts. Tobi Grundgeiger and his team noted this:

Ding et al argued a benefit of #AugmentedReality laryngoscopy was improved intubation ergonomics.

Inoue et al state "it may not be accurate to claim that all users adopt a forward-leaning posture".

This speaks to trust and the degree of automation. A good analogy is that of a self driving car. A self driving car and a purely manual car might be safer than partially autonomous vehicles as the purpose of the automation and trust in it is unclear
