What is this assault on physicians? Be willing to staff appropriately. Let an IM or FP or EM PHYSICIAN take a bronch course and cover the ICU before an NPP!!

Plan to continue the Doxy for three months. Bactrim is generally more noted as primary with Doxy as alternative therapy for eradication, the taxes are much more benign, chronic drug than Bactrim.

Ceftazidime may work for uncomplicated pneumonia, would recommend Merrem plus Doxy if concern for shock with AMS to get better CNS penetration.

So from that standpoint, take it with a grain of salt. But I would be concerned if you start seeing a typical pneumonia that are not responding to our typical CAP/HAP coverage.

If so, throw on your BDLS hats and notify those responsible in your chain. I’d be most concerned, obviously for Texas at this moment. I am not and have never been, however, a disaster response physician, or Director.

Yes, #melioidosis can be used as a bioterrorism option. But as I mentioned in the text messages, there was one or a small number of cases from the Midwest (Ohio?) a few years back (2021?) linked to an essential oils product out of Walmart imported from Southeast Asia.

Flucytosine dosing in cryptococcal meningitis:

FLOOR trial = single-arm trial evaluating reduced flucytosine dosing (60 mg/kg in 3 divided doses x10d)

Outcomes were inferior compared to historical data using standard dosing (100 mg/kg in 4 divided doses x14d)

We're stuck with full dosing #IDsky

It’s also the cost of the test that needs to be considered. It’s about $600 out of pocket, and ~ $400 is knocked off by coverage. So, yes it’s minimal patient discomfort, but it is not without its cost.
