Out of over 20,000 FDA-approved drugs, only 73 have active REMS—additional requirements that can make medication harder to get.

So it raised flags when the FDA put a REMS on Nexplanon, a birth control device available since 2011.

The FDA Quietly Slapped More Restrictions on the Birth Control Implant
The agency recently approved Nexplanon for up to five years of use—but it also imposed extra regulations that leave some providers worried.

The FDA recently approved the birth control implant Nexplanon for use up to five years, instead of three. But to providers' surprise, it also placed the device under a special monitoring protocol called REMS, which is infamous for the way it has limited access to medication abortion.

The biggest barriers to reproductive healthcare often sound inane and boring, which helps them fly under the radar. Behind this snooze of a regulatory change is a long history of paternalism at the FDA, including and perhaps especially under Democratic administrations, in fact:

Don't sleep on this wonky-sounding story. An expert told Garnet this birth control news is "troubling" and asked “What else is this going to be applied to going forward? Is every reproductive health product going to be subject to this layer of scrutiny?"
NEW: Trump's FDA recently approved Nexplanon for five years of use, but it quietly added extra regulations that leave some providers worried about access. The agency imposed a REMS program that only exists for 73 other drugs—including medication abortion.

Many providers did not yet know about the REMS when I contacted them.

Experts expressed concern about the agency's lack of transparency.

The decision to require a REMS for Nexplanon was made after Trump took office. However, the FDA also decided to require a REMS for a new IUD under Biden. It's unclear whether the Nexplanon decision was political, or whether the agency has changed its approach to regulating certain contraceptives.

Nexplanon has been on the market since 2011. The FDA can require a post-approval REMS when it becomes aware of "new safety data," but the agency would not specify what new safety data its decision was based on. According to Organon, its clinical trial revealed no new safety information.

Many providers had not yet heard about the Nexplanon REMS when contacted by Autonomy News. Nexplanon's maker, Organon, did not respond to multiple requests for comment.

Other outlets had reported some of the allegations against former PPSE interim CEO Mairo Akposé, but @susanrinkunas.com uncovered new details, including that Akposé wanted to halt abortions. PPSE's board had cleared Akposé of wrongdoing, but she stepped down not long after we published this story 👀

Thank you to @autonomynews.co for highlighting my article re pharmacists having a more free for all for dispensing certain medications used in abortion.

As a chronically ill person, scared also for others on methotrexate at this very moment.
There was so much news last week, including the little-noticed nixing of a rule that required pharmacies to carry and dispense misoprostol and methotrexate. More in our roundup:
