In my Ghosts of Saltmarsh campaign, took inspiration from the bear in the Annihilation movie and had the Vine Blights ( a random encounter) react to the players with the last words and voices of their victims but in a raspy approximation

For The Wine Dark Sea, I wanted a background that had a necromantic feel so first I thought about a background where you died and came back as you arrived in the Wine Dark. It felt odd since a background in the game is what you did before arriving. I pulled some inspiration from the Ancillary books
