👍Fantastic study 👀 at whether #shorterisbetter in Neutropenic Bacteremia in adults. >80% GNB with almost 18% MDRO, profound neutropenia in 83% of enrollees & post therapy Abx PPX in 65%. Adequate source control in 70%-100%. No diff in relapse or ☠️ if <10 vs > 10 ABX days. #IDSky #TIDSky

The preliminary sequence from the H5N1 human case in British Columbia has been posted and it is not good news. The virus potentially has a quasispecies at HA residue 226 (H3 numbering). This is bad news because we know that mutations at residue 226 can increase binding to human receptors. 1/

Health officials in California have confirmed the first case of mpox clade I in the US. Patient is a returning traveller's who traveled to eastern Africa. This is not unexpected because the world is interconnected and there are ongoing outbreaks of clade I mpox in Africa.

We have found the opposite 2 be true during this 👶outbreak. Incorrectly obtained NP swabs (not reaching the OP) have been false (+) 2 begin w/ only to turn (+) when retested in the PICU. And kids w/ progression on empiric S.pneumo, GAS & Staph coverage have ultimately required Azithro + steroids.

We often advocate for a unifying diagnosis (Occam’s razor), but in people with immunocompromise, it’s important not to disregard Hickam’s dictum; “Patients can have as many diseases as they damn well please!”

The US confirmed #H5N1 #birdflu count is now 52, from 7 states. 58% due to exposure to infected cows, 40% to infected poultry. The mystery Missouri case rounds out the mix. To date all cases have been mild. www.cdc.gov/bird-flu/sit...
