Maybe open referrals from District Nurse / UCR teams who will be able to name the people in their communities who would benefit from CGA type review?

Currently:
1. Reaching out to GP/AHP
2. Triaging general medical referrals so we see older ppl.
3. Extending offer to see appropriate ppl referred to other specialty eg colorectal/cardiology.
4. Great conversation with ENT colleague today. Plans to divert older people referred with 'instability'.2/2

Something about a stitch in time perhaps 🤔
The knock on transferred cost is exactly what keeps poor people poor. But those who run the pot hole budget are only going to be interested in their budget - they don’t pay the NHS costs for the A&E attendance. It’s silo working and blinkered.

I love the #QI model #15s30m to implement low energy high impact changes to processes.
