Nah that would have been registered before her arrival. At this point, the gov know she's trans. It's very much a controlled, scripted visit. She's the daughter of a noble and has diplomatic clearance for a visit regardless of her medical history.

The nation in question is an analog of fully realized far-right theocratized America. The Holy Eyes are everywhere. Magic ...cameras? that record actions, convictions, and intentions. They are aware if you are aware that you are breaking a law.

One idea for one of these scenes is to have them visit a restroom together and at the last minute, she breaks off to use the other one. When MC asks why, she says something like "I could be arrested here if I didn't" and that's when the reader starts getting clued in and hopefully thinking 3/

The goal is "this character happens to be trans". She's my MC's best friend and slow burn. As they adventure together, they interact with some cultures that are less progressive and sometimes downright hateful. It's in these places that some of her less obvious character details begin to surface. 2/

It's high fantasy sci-fi and the character cames from a society which is progressive and inclusive. It's intended to illustrate how society should be regarding this subject. Nobody really acknowledges it, not because it's secret but because it's not noteworthy. 1/x

That image is without a shadow of a doubt AI generated. If you found this on a site which claims otherwise, then I they misrepresented this image.
