Yes, it’s custom. My lower legs are really sensitive and I don’t like anything touching them. I’m trying to desensitize myself, but it’s not going well. :-(

I’m up about 50 lbs since having the first one done 20 years ago. The surgeon is only using BMI to exclude me from surgery. 😡
