This is a learned skill, and not intuitive to those going through the transition. I had to take a step back early on in my graduation to supervising physician (i.e. an attending) and realize I'm not really as funny as I think I am, these trainees and others feel obligated to laugh at my jokes.

I'll rephrase based on a decade of working in the field, it's natural to think everyone should get medical care, but it's also natural to feel that my injured arm is more important than a stranger's chest pain. It's why we don't let people triage themselves and why EMTALA is helpful for everyone.
