vikram is right. in order to show democrats that you don't like their healthcare policies you need to endorse their healthcare policies by voting for democrats
If you want universal health care, you have to elect people who will implement it. If sit on your hands and say “I won’t be bought cheaply; first implement universal health care and *then* I will support you”, everyone will correctly ignore you because you are a non-voter now
