Massive mechanistic blind spot: all diabetics have hyperglycemia but need diff drugs for type 1 vs 2 (we know why), but psychiatric drug treatment works only when the drug mechanism happens to match the person's unknown pathophysiology. This dilutes effect sizes in clinical trials.
