The Times Square Cup is a really cool event that Borussia Dortmund and Street Soccer USA have been partnering on for a few years now to raise funds for Street Soccer USA’s work with underprivileged and homeless youth. Wish I could be there!

Juventus vs Benfica has definitely been a good opener for this season’s UEFA Women’s Champions League, so far! 1-1 at the break, Juve’s had more possession and chances, but Benfica has been good on defense and dangerous on the counterattack.

In Hinchcliffe vs Football is for the Fans, Scott Michaels, things are moving apace. Amongst a number of other filings, Michaels asked for pro bono counsel.

The court denied the request basically because Michaels hasn't presented a defense that would warrant the assistance of an attorney.

I’m watching Deutschland ‘83 on Prime Video (great show, like a German take on The Americans, kind of), and it has Major Tom by Peter Schilling as the opening credits music. The subtitles identified the song as Blue Monday by New Order. 🤦🏼‍♂️

Rural hospitals keep closing, though, and added to the failing retail pharmacy chains, this is going to make access harder and harder for millions of Americans. This is the mess we’re in now, and it’s hard to see it improving, because it’s not really economically viable. 11/end

Hospitals. (Some) hospitals are always open, and this will be yet another thing added to their plate by various market forces and governmental decisions. Of course they are also affected by reimbursement problems, only even more so. They can’t count on selling you stuff while you wait. 10/11

Patients have to hope that they don’t need pharmacy services urgently and/or at odd hours. Of course, that’s not how health works! So when people need medication and can’t wait for the mail, or the big box or clinic pharmacy is closed, what option do they have? 9/11

So the big chains start to reduce hours, then close stores, then shutter completely. The formerly ubiquitous, convenient pharmacies aren’t around anymore and there’s nobody to step into that void. Where does that leave patients? 8/11

Now there were only a handful of pharmacy chains that provided a full range of services and were open late hours, but that got harder and harder to sustain economically, so they cut back. Cut hours, cut staffing, lagged in keeping up with wage competition from other retailers. 7/11

Insurers/the government don’t just pay providers for the cost of your prescription minus the copay you paid. The insurers/government decide how much they’re willing to pay and for which drugs/services and providers have to take it or leave it. 6/11

In addition to that competition from insurer’s mail order pharmacies and big box retailers, reimbursements are always getting worse and drugs themselves are getting more expensive. What do I mean by reimbursements? 5/11

Insurance providers bought or started their own mail order pharmacies and increasingly drove their users to these services through pricing and coverage incentives. Meanwhile, big box retailers and grocery stores heavily pushed discount prescription services at a loss to drive foot traffic. 4/11

While they were building and competing with each other, there was at least the demand to provide lots of services. 24-hour pharmacies were common. The 2008 crash and competition from big box stores and insurance companies started to put the hurt on the big pharmacy chains, though. 3/11

First, let’s look at how we got here. Throughout the 1990s-2000s, the large pharmacy chains consolidated through buying up their competitors and building up near ubiquitous presence in the markets they dominated. Independent pharmacies fell first, then smaller regional chains. 2/11
