Another excellent article by Tara Haelle. I’ve spoken to her a number of times for previous articles and she is one of the best journalists out there covering pediatric medicine. This is a complex issue and she deftly handles the controversial aspects of abusive head trauma.

Dear Director Bhattacharya:

I neglected to include a couple of important points in my earlier email.

First, I am glad that you raised the issues about the liver toxicity of acetaminophen. Acetaminophen is an important drug because it is a useful analgesic and antipyretic without the bleeding risks of NSAIDs such as aspirin and naproxen. However, the liver toxicity effects are quite important. It is one of the leading causes of serious acute liver injury in the United States, particularly when taken at high doses. Moreover, it is unusual for a widely used drug in that its mechanism of action is not well established. It is believed to act by inhibiting cyclooxygenase specifically in the brain, but it has not been possible to demonstrate this conclusively.
