I have prescribed zofran for upset stomach, propranolol for akathisia, all sorts of stuff

that's why it's not a clear strict line between the two, there's a bunch of crossover

but the problem is what role they're functioning in more than what they're prescribing, to me

I've heard of this as a potential side effect of antipsychotics, but what I took away was it was like an uncontrollable tremor.

Thank you for the detailed description of akathisia. This sounds much worse than I'd guessed it was.

i find this stuff interesting, but i'm not an expert in how medications work, and we don't know 100% what's going on with akathisia either way. just that it happens MOSTLY with antipsychotics, along with several other movement disorders: tardive dyskinesia, parkinsonism, and dystonia

*you CAN get akathisia from going OFF of stimulants, if your receptors have been keyed to having stimulants in your body for a long time.

this is my very VERY basic layperson understanding, anyway, do not quote me in the newspaper here lmfao

(that is a very VERY "i'm not a scientist or doctor" description, btw lmfao. HUGE GRAIN OF SALT HERE) so stimulants, which have the opposite effect on dopamine, are sometimes used to TREAT akathisia, actually. you can have stimulant-induced activation, but it's not the same thing exactly*

to clarify, i'm not saying that every other movement-related or activation-oriented experience is LESS BAD than akathisia, necessarily. but it's a very particular thing that happens due to a deficit in dopamine (which is why it's present in antipsychotics and parkinson's)

i think the best way to describe akathisia is that it's VERY similar to severe nausea. it's like a full body rejection of your own full body. like having ants crawling both inside and outside your skin. it's torture! it can lead to suicidal ideation in very short periods of time unfortunately
akathisia (a couple people asked haha): basically a physical inability to remain still. moving is compulsive, and often frantic; the need to move is intense and distressing. people end up pacing for hours on end. it can lead to a feeling of impending doom. antipsychotics are a particular trigger

this is probably oversharing but GOD i hate trialing new meds. with most stuff it's like eh, nothing happened, or eh, a couple days of nausea, maybe headache, etc. with antipsychotics and mood stabilizers it's always like *monster car announcer voice* GET READY TO GET FUCKED UUUUP

it's hard to explain tbh if you haven't experienced it. if it helps, akathisia is almost always a side effect of medication though, afaik it's not like a condition that you just develop on your own (outside of a movement-related disorder like parkinson's, anyway)

for mania, it would have to last 7+ days, and be acting overtly out-of-character (among other symptoms). adhd makes you fidgety/restless, but akathisia is more like...you have to move so badly it makes you want to throw up, and moving doesn't make you feel better, but you have to do it anyway haha

apparently about a quarter of people with schizophrenia have chronic akathisia, which is AWFUL?? anyway it really, really really sucks lmao. probably my least favorite common side effect of my meds after severe nausea (although...there are similarities in the full body sensation of NOPE lmao)

on a new antidepressant that is working well except it's giving me akathisia and i can't decide yet if it's worth it. i've had restless legs nearly my whole life but now i feel like i have restless entire body and it's a very strange feeling

I've taken 2 blood pressure meds in the last 24 hours and still have mild akathisia, so I guess I will wake up and start my day at 4:41am.
