In the meantime, people who are specifically trained to provide evidence-based patient counseling services, are rarely incorporated into the proverbial 'multidisciplinary teams'. Because the 'system' is not built for that.

While authors use careful language, aficionados r already rejoicing. Massive threats to causal inference, res. confounding, colliders, etc...., most in direction of overestimation.To separate signal from noise we need serious causal thinking-driven research & edu.

IFM in its best form is practicing off rodent studies &underpowered trials measuring non-validated surrogate risk factors. It centers provider enthusiasm abt their favorite mechanism & high risk of bias data over providing real patient centered care that communicatess honestly what we know/dont

We know the drivers of research waste in academia are

⚠️Pressure to maximize papers and PhD students
⚠️Endless demands on time due to poor management
⚠️Stakeholders don't insist on robust quality systems to underpin mission critical work

Solutions that don't address these are pointless.
