This is a powerful account from a PhD student whose fellowship was terminated by NIH because it was part of a "diversity" program without any other considerations or understanding of what the program or the students were actually doing.

Pre #nephjc bias: Team pro protein

All these studies are deeply confounded and based in the pre-kidney-preserving therapy era.

Let them eat steak!

(Actually, let them eat vegetable sources of protein, but beans, lentils, and soy don’t rhyme with cake)

Loved watching @kidneyrebel.bsky.social grand rounds at @nu-nephrology.bsky.social!!

Thank you for the PKD mega blocks shout out #nephjc (1), happy to be at the right overlap in my Venn diagram (2), and trying to build toward a medical specialty in a hill (3)

My op-ed just came out discussing how NIH leadership is failing early career researchers and, importantly, how we can push back and take our future into our own hands.
NIH Leadership Is Failing Early Career Researchers
Halted training programs, funding chaos, and hiring freezes jeopardize advancement options for early career researchers.

This year's Nobel Prize in Physiology or Medicine was just announced:
www.nobelprize.org/prizes/medic...

This seems like a good opportunity for me to explain a bit about peripheral immune tolerance and where the prizewinners' work comes in 🧵
6 October 2025

The Nobel Assembly at Karolinska Institutet has decided to award the 2025 Nobel Prize in Physiology or Medicine to:

Mary E. Brunkow
Institute for Systems Biology,

“for their discoveries concerning peripheral immune tolerance”

Agree 100% that maintenance dialysis pts and post transplant pts deserve better quality of evidence than we have.

People’s practice habits are so hard-wired in training.
Lisinopril and losartan are still the most prescribed ACEi and ARB.

Right: more aldo, more combo meds, advocating for GLP1 as part of lifestyle change
Wrong: 130=/=120, even stronger aldo rec, even stronger GLP1 rec so that insurance will cover it

If someone feels bad on multiple different agents tried serially to get them below 140 that’s a special case.

I do wonder if there’s an argument to look for carotid stenosis or similar in patients like this.
