I am very lucky to have the mentorship of Andrew Davis, Massimo Cristofanilli, Aditya Bardia, William Gradishar, and Cynthia Ma, among others. They have supported me every step of the way and I am forever grateful to them and all the incredible people who worked on this project with me.

What do we do now that we have identified this disparity in targeted treatment use? We will evaluate the potential root causes and design interventions to address this gap. In the era of precision oncology, we must ensure patients equitably receive the recommended ctDNA-guided treatments. (5/5)

Despite treatment advances, we also found that Black patients with metastatic breast cancer continue to have shorter survival compared to White patients, measured from time of ctDNA. This reflects the persistent and striking nationwide racial outcomes disparity in breast cancer. (4/5)
Two Kaplan Meier curves that show that Black patients have a shorter survival after ctDNA testing compared with White patients. Black patients with progesterone receptor negative metastatic breast cancer have the shortest survival.

We found that Black patients with metastatic breast cancer were significantly less likely to receive PI3K inhibitors - the medications that can be given to patients with PIK3CA variants - than White patients (5.9% vs 28.8%). This disparity was despite the same % of PIK3CA mutations. (3/5)
podanymd.bsky.social
We found racial differences in variant frequency - higher rates of GATA3 and CCND2 variants in Black patients with MBC - by ctDNA which were validated in a large real-world cohort. But no differences in the targetable mutation PIK3CA, so we studied treatment equity next. (2/5)
A graphical depiction of differences in genomic findings between Black and White patients with metastatic breast cancer. It shows that Black patients have a higher rate of CCND2 and GATA3 variants. A graph depicting racial differences in ctDNA findings in a large real world cohort. There are many differences, but it supports the finding that Black patients have a higher rate of CCND2 and GATA3 compared with White patients.

✨My essay in @thelancet.bsky.social this week has made the front page!✨ (I didn’t know about this 🤗)

‘Reflect, Collaborate and Listen’ looks at why doctors don’t listen and the urgent need to rebalance the power dynamic in the patient - doctor relationship.

I've decided that when researchers say a treatment is "well tolerated" it actually means "I can tolerate my patients having toxicity" and nothing to do with what the patient thinks. We can only make such a statement if we use patient-reported outcome or experience measures.

Cervical cancer mortality decreased 15.2% per year from 2013-2015 to 2019-2021, suggesting HPV vaccination has affected the sequential decline in HPV infection prevalence, cervical cancer incidence, and cervical cancer mortality.
