Hideki Kamiya created Devil May Cry - and with it, codified Stylish Action games - after playtesting Onimusha: Warlords and finding a bug wherein enemies could be juggled in the air with repeated sword strikes. He thought that shit was so cool, he'd make an entire game based around that.

Phantasy Star III has a soft-locking bug very early in the game: if you use an Escapipe to leave a jail cell before another character breaks you out, the guards at the town's entrance will never leave, trapping you in the starting town forever. An NPC even breaks the fourth wall regarding this.

Sonic the Fighters began as some AM2 employees sneaking models of Sonic and Tails into Fighting Vipers for fun. Studio head Yu Suzuki saw this, thought it looked cool, and managed to secure permission to make a Sonic fighting game from then-Sonic Team president and future convicted felon Yuji Naka.

There are two demons in Persona 3 called Cerberus: the Persona used by Koromaru, and the classic depiction of Cerberus from throughout the Shin Megami Tensei series.
The latter Cerberus cannot be obtained in normal gameplay.
