At the risk of being very un-opinionated, I think it would depend on intent/who said it - and how - as someone clumsy with words might actually think it's a compliment despite it seeming backhanded. A long pause and a raised eyebrow would make me think they meant to be a bit arsey.

At the risk of sounding defeatist what struck me most was that even with basically unlimited resources and with someone who really wanted to be well it was just too much. It made me feel very humble about the things I take for granted.
