Two plant-based protein powders contained enough lead that our experts advise against consuming them. Naked Nutrition’s Mass Gainer powder contained 7.7 micrograms of lead per serving, which is roughly 1,570 percent of CR’s level of concern for the heavy metal. One serving of Huel’s Black Edition powder contained 6.3 micrograms of lead, or about 1,290 percent of CR’s daily lead limit. Two other powders contained lead between 400 and 600 percent of CR’s level of concern: Garden of Life’s Sport Organic Plant-Based Protein and Momentous’ 100% Plant Protein. Consumers should limit these to once a week, Akinleye says. (See company responses below.) The only non-plant-based protein powder with lead detected at over 200 percent of CR’s level of concern was MuscleMeds’ Carnivor Mass powder. Six additional plant-based powders, five dairy-based powders and shakes, and one beef powder contained lead above CR’s level of concern. We also found measurable levels of cadmium and inorganic arsenic in some products. One serving of Huel’s Black Edition plant-based protein powder contained 9.2 micrograms of cadmium, more than double the level that public health authorities and CR’s experts say may be harmful to have daily, which is 4.1 micrograms. Another plant-based option, Vega’s Premium Sport powder, had enough cadmium that one serving would also put you just over that level. In one dairy-based product, Optimum Nutrition’s Serious Mass whey protein powder, we also detected 8.5 micrograms per serving of inorganic arsenic, which is twice the limit of what our scientists say is safe to consume daily.

the results of this investigation surprised me. since CR last tested protein supplements in 2010, protein mania has swept the nation. protein isolates (the key ingredient in supplements) are in everything these days, from pasta to starbucks cold foam

but as the industry has grown, so have the risks
Why We Tested Protein Powders and Shakes. Since our last tests of protein powders 15 years ago, protein has become the centerpiece of the wellness craze. Heavy metals were widespread in protein supplements back then, and today, they’re even worse.

About the PURPOSE Program Gilead's landmark PURPOSE program is the most comprehensive and diverse HIV prevention trial program ever conducted. The program comprises five HIV prevention trials around the world that are focused on innovation in science, trial design, community engagement and health equity. The PURPOSE trials are evaluating the safety and efficacy of an investigational, twice-yearly injectable medicine, lenacapavir, to reduce the chance of getting HIV. The Phase 2 and 3 program, consisting of PURPOSE 1-5, is assessing the potential of lenacapavir to help a diverse range of people around the world who could benefit from PrEP. More information about the PURPOSE program, including individual trial descriptions, populations and locations, can be found at www.purposestudies.com . More data from Purpose Alongside its Phase Ill plans, Gilead also presented more data from its Purpose clinical programme at the California conference. This included data from adolescents, aged 16 and 17 who were enrolled in the Purpose 1 study, with lenacapavir plasma concentrations being comparable between adolescent and adult trial groups. The therapy was also able to show 100% no incidence of HIV infections across the adolescent group, the same as the adult group in the same study. Participants in both groups experienced the same most common AEs. Gilead now intends to submit data to agencies to support the use of twice-yearly lenacapavir in adolescents. Gilead also revealed quantitative and qualitative survey data from the Purpose 1 trial, with approximately two-thirds of survey respondents preferring twice-yearly lenacapavir compared to once-daily pills. Additionally, 61% of respondents reported they would feel more protected from HIV with twice-yearly PrEP injections compared with once-daily pills, and 61% of respondents also reported they would feel more confident about not missing a PrEP dose with twice-yearly injections.

Medication-emergent adverse events were similar between the two cohorts and mostly mild to moderate in severity. The study was designed to evaluate the safety and pharmacokinetics (PK) of the therapy. NOTE. Participants had to be found to be negative for HIV prior to the trial.
New PrEP modality preference data demonstrate preference for twice-yearly lenacapavir vs. once-daily orals in PURPOSE 1 survey participants New quantitative survey data and late-breaking qualitative survey data from the PURPOSE 1 trial, evaluating twice-yearly subcutaneous lenacapavir for PrEP among cisgender women in sub-Saharan Africa, were also presented at CROl in two poster sessions. An interim analysis of surveys included 2,561 trial participants' self-reported preferences for twice-yearly lenacapavir for PrEP injections and once-daily pills, with approximately two-thirds of survey respondents preferring twice-yearly lenacapavir at Week 52 of their trial participation, compared to fewer than one-third of respondents preferring once-daily pills. Additionally, at Week 52, most respondents (61%) reported they would feel more protected from HIV with twice-yearly PrEP injections compared with once-daily pills, and 61% of respondents also reported they would feel more confident about not missing a PrEP dose with twice-yearly injections compared with once-daily pills. Qualitative data among 108 participants also showed that respondents, particularly adolescents aged 16 and 17 years, noted that twice-yearly injections better suited their lifestyles compared with once-daily pills. New study population data from PURPOSE 1 show comparable pharmacokinetic and safety profiles for both adolescent and adult trial participants Additional adolescent-related data from the PURPOSE 1 trial were also presented at CROl yesterday during an oral abstract session and press conference. PURPOSE 1 is the first adult Phase 3 HIV prevention trial to intentionally include adolescents aged 16 and 17 years, and trial enrollment was much higher than in typical adolescent-dedicated studies (124 adolescents, 56 of whom were assigned to the lenacapavir group). The data showed that observed lenacapavir plasma concentrations were comparable between adolescent and adult trial groups, with participants in both groups experiencing the same most common adverse events. There were zero incident HIV infections across the adolescent and adult groups receiving lenacapavir. Given these results, data submitted to regulatory authorities support the potential use of twice-yearly lenacapavir for adolescents who need or want PrEP The use of lenacapavir for the prevention of HIV is investigational and has not been determined to be safe or efficacious and is not approved anywhere globally. There is currently no cure for HIV or AIDS.

and About Lenacapavir
Mechanism of Action LEN is a novel, first-in-class, HIV-1 capsid inhibitor with a multistage mechanism of action and a desirable in vitro profile. The HIV capsid protein is part of the group-specific antigen (gag) region of the HIV-1 gene and plays multiple roles in the HIV replication cycle. LEN binds to the viral capsid protein and prevents nuclear transport, virus assembly and release, and capsid assembly in new virions. The sustained duration of LEN release from the injection site and its low systemic clearance (in vivo) make LEN an attractive candidate for a long-acting HIV therapy. Additionally, LEN exhibited synergistic effects and remained efficacious against resistant HIV-1 variants in vitro when combined with other antiretroviral therapies. About Lenacapavir Lenacapavir is approved in multiple countries for the treatment of adults with multi-drug resistant HIV in combination with other antiretrovirals. The use of lenacapavir for HIV prevention is investigational and the safety and efficacy of lenacapavir for this use have not been established. The multi-stage mechanism of action of lenacapavir is distinguishable from other currently approved classes of antiviral agents. While most antivirals act on just one stage of viral replication, lenacapavir is designed to inhibit HIV at multiple stages of its lifecycle and has no known cross resistance exhibited in vitro to other existing drug classes. Lenacapavir is being evaluated as a long-acting option in multiple ongoing and planned early and late-stage clinical studies in Gilead's HIV prevention and treatment research program. Lenacapavir is being developed as a foundation for potential future HIV therapies with the goal of offering both long-acting oral and injectable options with several dosing frequencies, in combination or as a mono agent, that help address individual needs and preferences of people and communities affected by HIV.

Today, we will discuss the ONCE-YEARLY results.
KEY POINTS • Last year, Gilead released data showing that an HIV drug, called lenacapavir, could provide virtually complete protection against infection with just a single injection every six months. •The drug, now under regulatory review, was greeted as a breakthrough, the closest thing the field has ever had to a vaccine. • Gilead published early data suggesting a new formulation of the drug could be used to prevent infection with just a single shot every year. • Gilead is set to launch a Phase Ill trial of its once-yearly lenacapavir pre-exposure prophylaxis (PrEP) for human immunodeficiency virus (HIV) following the success of a Phase I study. • Lenacapavir stops the virus from replicating inside cells. • If future trials go well - now it has passed the first, Phase I, testing hurdle - it could become the longest-acting form of HIV prevention available. •Currently, people can take daily pills or sometimes have injections every eight weeks, for pre-exposure prophylaxis (PrEP), to reduce their risk. •The Phase I trial investigated two single 5,000mg doses of the yearly injection in 40 healthy participants at low risk of acquiring HIV. One formula contained 5% ethanol, the second contained 10% ethanol. •Both groups achieved and maintained plasma concentrations above the 95% efficacy threshold for 56 weeks. IMPORTANT NOTE. The study did not actually test efficacy in preventing HIV. Instead, investigators injected two groups of 20 volunteers with two different proposed formulations and took regular blood samples to measure the levels of drug that remained in the blood. Then they compared those levels to drug levels from volunteers in the two large trials. Sure enough, volunteers who received either new formulation had higher levels of drug in their bloodstream for 56 weeks than volunteers receiving once-every-six-month injections. That suggests the new medicine should provide equal protection.

KEY POINTS • Gilead's twice-yearly shot reduced HIV infections by 96% in a second large Phase 3 clinical trial. •99.9% of participants who received Lenacapavir did not acquire HIV, with two cases among 2,180 people. • The trial included cisgender men, transgender men, transgender women and gender non-binary people who have sex with partners assigned male at birth. •There were nine cases of HIV in a group of more than 1,000 people assigned to receive Truvada, Gilead's older daily pill used for prevention and treatment. • Lenacapavir was 89% more effective than Truvada in the study. • Lenacapavir and Truvada were also generally well-tolerated by patients with no new safety concerns, according to Gilead. • Gilead plans to present detailed data at an upcoming medical conference. • Gilead in June also said Lenacapavir was 100% effective at preventing HIV in another late-stage trial with cisgender women. • None of the approximately 2,000 women in the study who received the shot had contracted HIV by the time of an interim analysis conducted in September 2023. •The data sets the stage for likely FDA approval of Lenacapavir for HIV prevention and launch in the market by 2025.
