Certainly not to all patients, but probably just to those with significant cardiovascular risk factors, I would say. If you need to give Lasix and it dips your K to low-normal then add spiro early #NephJC

Even the NEJM editorial comment on RALES argued that spiro reduced sudden cardiac death by arrhythmia by blocking urinary K loss (MRA), this being a superior way to maintain total body stores of an intracellular ion than replacing losses. #NephJC

Keep in mind that there is the argument that blocking urinary K loss (MRA) is a superior way to maintain total body stores of an intracellular ion than replacing losses. Extracellular serum K levels reflect total body stores poorly.
