Bradycardia on arrival at the ED appeared to be a positive predictor of xylazine detection among patients with opioid overdose, a study suggested.

This marks the first time that researchers have tied this symptom to the novel adulterant.

Clinical pearl: opioid overdose + unexpected bradycardia should raise suspicion for xylazine exposure. Naloxone still treats opioid toxicity, but it will not reverse alpha-2 agonism. Expect atypical physiology.

Key signal: xylazine detection clustered in the Northeast and was independently associated with bradycardia on arrival. Not hypotension. Not hypoxia. A specific physiologic fingerprint worth noticing.

Let’s add to it.

Different doctor. They prescribed a medication. I need a preauthorization. They won’t do it unless I come in and see them.

So I need the medication. They prescribed it. And if I could pay out of pocket I could avoid Bradycardia. But if not then I have to come and see them.

Heba Muraisi is in critical condition on day 72 of her hunger strike. She struggling to breathe and suffering uncontrollable muscle spasms.

Kamran Ahmed is on day 65. He is experiencing shrinkage to the muscle of his heart and has been diagnosed with bradycardia.

Bradycardia, or abnormally slow heart rate, has been identified as a clinical sign associated with xylazine-fentanyl overdose, particularly in the Northeastern United States.
Abnormally slow heart rate associated with xylazine-fentanyl overdose
Researchers have identified bradycardia—an abnormally low heart rate—as a symptom of xylazine-opioid overdose. This breakthrough finding from the Icahn School of Medicine at Mount Sinai may help emergency medicine physicians detect whether patients have been exposed to xylazine, a drug that is increasingly found as an additive to the illicit fentanyl supply, particularly in the Northeast.

Kamran Ahmed, has gone more than 60 days in a hunger strike for Palestine and against his imprisonment. On January 7, he was hospitalized for the sixth time after doctors diagnosed him with shrinkage of his heart muscle and severe bradycardia, placing him at imminent risk of sudden cardiac arrest.

Transcutaneous pacing (TCP) is an intervention for unstable bradycardia. Electrical stimulus is provided ~1/s to increase heart rate. With transvenous pacers and chronotropic meds, TCP is mostly limited to the prehospital setting and low-resource centers.

We suck at TCP. #emimcc #medsky #cardiosky

This results in a slower heart rate (bradycardia), reduced blood pressure, and a feeling of calm, which helps manage panic, anxiety, and emotional overwhelm. It's a key component of Dialectical Behavior Therapy for distress tolerance.

The heart muscle itself begins to break down, leading to a slow heart rate (bradycardia), heart arrhythmias, and sudden cardiac failure.

By day 60, damage to the brain, liver, and kidneys is often permanent, even if the person resumes eating.
