I find in the survey for my book & related work that the most common reason why people do not appeal coverage denials is they do not know it is an option available.
The 2nd most common reason they don't appeal is they don't think people stand a chance against UHC, Cigna, etc. when it's a coin flip.

knowing what we know now about the efficacy of the vaccines, the strain on hospitals, and the mortality of vaccine refusers in the Delta wave the "let 'er rip" approach advocated for in the Great Barrington Declaration would've killed literally millions of additional Americans, for no point at all.
I'm writing a theoretical review paper on a subject I knew nothing about a few months ago, and I think, yeah, I could have been a theoretician.
