I know everything is crazy but let's celebrate some wins. I diagnosed a patient with new onset diabetes and heart failure last June, initial A1c 14% EF 21%

As of last week, EF has improved to 35% A1c is 6.2% (!!!!) and patient has lost about 40lbs to a near normal BMI. (Tirzepatide + flozins!)

I think this data is interesting and mildly reassuring.
I feel like #flozins are fairly benign (no safety signals), but do they work in transplant patients like CKD patients…that remains to be determined #NephJC

Also because #flozins do not seem to follow any of the other mechanistic principles as in CKD patients, it seems scary to just accept the eGFR dip as due to them alone. We would end up evaluating for other causes. #NephJC

The 1° endpoint was seated systolic blood pressure—chosen as a surrogate of vascular and volume effects.
Is this a good endpoint given the short length of the study?
Do you usually see ⬇️ in BP in patients on #flozins? Do you frequently stop BP meds?

My clinic recently got an embedded pharmacist. She's made it her mission to get as many folks off sulfonylureas and on to flozins and GLP1s simply for the morbidity/mortality benefit alone. She also helps manage our tough HTN and HF patients. She's been a god send for chronic disease management
One of the keys to expanding the scope of primary care is inclusion of really-good pharmacists.

As a PCP I've more or less taken over care of my CKD/DKD patients as my local nephros don't seem to do a great job of managing unless they're approaching dialysis.

We have all the tools now. ARB, flozins, GLP1, MRA etc that it's not that difficult to manage most of them
CKD care needs to improve and we have options now to do so!

DKD care could be done by primary care with our help/guidance in tricky situations

GN diagnosis (esp IgA, APOL1) needs to increase since we have (or will have)!options to treat these
