Many if not most of the first trans men who got bottom surgery in the UK after WW2 got it by pretending (typically with at least the tacit approval of the surgeons) to be cis men with injuries or developmental abnormalities.
The irony is that virtually all trans healthcare treatments are minority by-uses of procedures developed for cis people. HRT for menopause, breast augmentation for cis women's cosmetic surgery, phalloplasty for men with war injuries, puberty blockers for precocious puberty cis kids, etc
