Encouraging signs that design and QI can work powerfully together in the NHS. This paper from NHS Tayside shows how design tools can support more person-centred, creative, and collaborative improvement.

Triage and decision making: having a head injury is frightening for patients and for those close to them. We should be doing our best to ensure they understand what is going on and what the options are. #RealisticMedicine #SDM @scarlettmcnally.bsky.social @trishaelliott.bsky.social 6/

We should do it much more!

I actually like “BRAIN” even better.

*Risks
*Alternatives?
*Instinct (patient & doc)
*Nothing (what if we do nothing)

This is a very helpful piece in the @bmj.com from @scarlettmcnally.bsky.social on improving shared decision making in medicine (here specifically, surgery).

When we talk about waiting list numbers, I’d be keen to know whether everyone on them has had a thorough review as described.

I’m enjoying reading “Sod Seventy” by Muir Gray.

About to start the chapter on choosing and using healthcare wisely…

Muir reminds us that, for multiple reasons, doctors themselves are cautious users of healthcare services.

As @ClareGerada highlights, gold-plated clinical guidance for ideal conditions is not practical clinical guidance for local conditions. The best care pathways support clinician and patient to agree what is relevant and achievable: https://t.co/4iUYkd2BPp

It all depends on what we value most. So many decisions are financial rather than what matters most.
The thing I love about #realisticmedicine is that when we do what matters most for patients, the evidence is we spend less.
It's all gone back to front in pathway driven care.

From the Chief Medical Officer:

“If we attempt … to design and promote a system of transactional care, we run the risk of losing the art of care - the human connection that makes care caring - and promoting moral injury to our staff.”

Good to see proton pump inhibitors (drugs like omeprazole and lansoprazole) discussed here in the British Medical Journal.

They aren’t harmless, in a multitude of ways, yet many people end up on them long term.
