I never give SU, certainly not in CKD peeps! But those folks who walk in with A1c 11 to PCP or endo? They should definitely get aggressive glycemic control with metformin and whatever injectable is affordable (unfortunately that is still insulin sometimes). SGLT2 as well!

SUs should be completely out for sure, but A1c lowering does have effects on hard outcomes (retinopathy, nephropathy) that we care about, and metformin and insulin remain extremely effective. For most of the patients we see in CKD clinic, I agree with you, but not so for primary care and endo.

I do think this is unfortunately part of the point. UPF are scientifically designed in many cases to maximize palatability, which likely leads to increased intake (along with higher energy density). I don't think that can be factored out of these experiments.

Seems like a myth. I really haven't seen absorption issues, per se, in the absence of overt GI symptoms like nausea or diarrhea. So I don't think any issues would be subtle. Either they are too sick to take meds or not, and if so, no more GLP1ra (or titrate more slowly, etc etc).

One would think benefits to the kidney from GLP1ra would be similar in transplant to those in obesity - the physiology is not dissimilar in that both are at risk of hyperfiltration related injury.

Great meds for pre-existing DM or NODAT. Our transplant nephrologists essentially "fire at will" with these. Obviously some more caution is warranted with GI side effects/inability to tolerate immunosuppression, but that degree of GI upset is going to be exception rather than norm.
