I just wrote a podcast about Flixborough. The repairs they made to a system carrying highly flammable chemicals were cavalier at best. 28 workers died, but it could easily have been hundreds. No one was prosecuted. In a H&S vacuum working people pay the price. podcasts.apple.com/gb/podcast/c...

I think very few H&S regulations are unnecessary.
There are those which stop lethal events happening, and those which stop minor-but-damaging events from happening.
When they work, you don't notice them.
When they fail, lives are ruined.
Corporate greed, however, appears to be insatiable.

TIL about a memorial ceremony in Iceland in 2019 to mark the end of a glacier, changing the place name from Okjökull to Ok (jökull = glacier). Uncompromising wording on the bronze plaque:
"This is to acknowledge that we know what is happening and what needs to be done. Only you know if we did it".
