Here’s a beautiful example in my neighbourhood from this weekend.
Driver establishing themselves in the bike lane was the previous design, called a dashed bike lane. The new design and new signs are very clear, drivers yield to the cyclist in the bike lane. Cross the bike lane in the intersection.

Most often just absolutely distracted. I watch drivers eyes and it's surprising how often they are gazing into infinity. Straight ahead.
