It wasn’t moderation specifically, but I’ve done high-volume triage and follow up using tools that were insufficient and designed for a completely different workflow. Everything takes longer than it should and mistakes are inevitable.
Real example, the moderation software doesn't allow images to be emailed (currently), so mods were typing a summary description of the image and alt text. Users then file appeals since it wasn't the same alt text that they posted, so it clearly wasn't a violation.
