This "real world" study in pancreatic cancer related VTE reported in a JCO meeting abstract found apixaban was associated with less bleeding, fewer strokes and lower overall mortality compared to rivaroxaban

Real-world effectiveness and safety of apixaban versus rivaroxaban in pancreatic cancer–associated VTE.
739Background: Pancreatic cancer is highly associated with venous thromboembolism (VTE), requiring long-term anticoagulation. However, evidence comparing outcomes between apixaban and rivaroxaban in t...

This case shows argatroban as a lifesaving bridge and a cautious transition to apixaban, despite recent subdural hematoma. Would you anticoagulate, or hold, in this scenario?

I share your concerns Louise. I'm on Apixaban and don't need a fall, which could have bad consequences. I brought the bins in earlier, there was white & black ice on the steps, so with -5degC forecast next couple of nights the ice isn't going anywhere fast, I'm probably housebound for a while yet.

I take apixaban and cloiodgrel, aspirin isnt the best for thinning blood either so not sure with his obsession with that
January 3, 2026 at 2:51 PM
Yes. Could be on both. Standard protocol for folks his age with any evidence of heart disease could include BP meds, anticoagulants (eg., apixaban), and blood thinners.

Because I’m not a doctor I’m allowed to say shit like, “it seems like the president may be having frequent acute coronary events and strokes and his docs might want to try more aggressive therapies” but yes please consult a physician before starting warfarin or apixaban without discontinuing aspirin
January 1, 2026 at 4:33 PM
Not a doc but I know a bunch of cardiologists and it’s absolutely true that if you have extensive cardiac issues (like a history of heart attacks or strokes) that you do want “nice thin blood” and yes aspirin can help that but higher quantity ≠ better and you should add warfarin or apixaban
