That can be with insurance, if it's subject to your deductible. Insurance may not cover it if its not on their formulary list.

They can’t put that in writing or else they’d be codifying illegal policy whenever they ask for PA on something that’s supposed to be filled no questions asked by say, state Medicaid formulary or whatever.

* I can find Rx coverage through "the formulary" but each insurer has eleventy billion of them and the one for your plan might NOT be the one your employer negotiated

People used to cede custody of kids to relatives so they could have healthcare.

My Life saving Actemera, (Vasculitis med) made in England by GENTEC, will now be dropped from every formulary in the country by any Medicare Advantage or otherwise insurer. As will my diabetes medications. I'm dead.

Maybe truly chosen at random, but minoxidil is an ancient hypertension drug that is now used for its side effect - hair growth. Not really sure it is what you want to make a case for- it’s basically a vanity med.

What pts really need is what the VA has- national formulary, just a copay 👍🏻
